The identification particulars of the sample village/block and the sample household in which the survey schedule have been can­vassed have been recorded in block 1. Certain characteristics of the sample household, such as, size, social-group, principal occupation, etc., meant to be used as classificatory variables have been recorded in block 3.

The demographic particulars like age, sex and marital status, of all the normally resident members of the sample household and the particulars of degree of disability, blood relationship between parents, living arrangement, gener­al education, etc. of all disabled members have been recorded in block 4.

This part of block 5 records for each disabled member the particulars of type of disability.

Block 5 records for each disabled member the detailed particulars of disability, such as, type of disability, degree of each type of disability, cause of disability, age at onset of disability, type of aid/appliance acquired etc.

The information as to whether or not a disabled child in the age-group 5-18 years was ever enrolled in a ordinary school/special school, whether the child is continuing in that school or not, reason for non-enrolment/discontinuation, etc., have been recorded in block 6.
