Interviewer instructions

Survey code : Whether the originally selected sample household has been surveyed or a substituted household has been surveyed will be indicated against this item by recording '1' if it is the originally selected sample household, and '2' if it is the substituted one. If neither the originally selected household nor the substituted household could be surveyed i.e., if the sample household was a casualty, code '3' would be recorded.
