Interviewer instructions

If the original enterprise is surveyed, code against this item will be 1. If the original enterprise could not be surveyed for some reason or other and some substitute enterprise had to be surveyed, code will be 2. If the substitute enterprise also could not be surveyed, code will be 3. In such cases, only blocks 0,1,11,12 and 13 are to be filled in and the word 'CASUALTY' may be written on the top of the schedule.
